Job summary

Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere (CAW) program to deliver in-home medical and social support to our seniors and chronically ill patients. The role focuses on preventing hospitalizations through timely clinical interventions and tailored care plans.

Responsibilities include in-home assessments, medication reviews, health screenings, patient education, and close collaboration with PCPs and specialists to optimize patient outcomes in the home setting.

Qualifications

Active Nurse Practitioner license or Physician Assistant license.
Active RN license and Furnishing number (Must, upon hire) NP only.
Active Nurse Practitioner or Physician Assistant board certification.

Responsibilities

Conduct in-home assessments and perform medication reviews and health screenings.
Provide patient education by aligning care needs with appropriate interventions.
Identify diagnoses to be assessed in care management and active medical management.
Coordinate with Regional Medical Officer and other team members to develop appropriate care plans.
Coordinate care with PCPs, specialists, and ancillary providers; lead the clinical team.
Lead broader clinical team which may include nurses, health coaches, social workers, and care coordinators.
