Job summary
Connected Healthcare Partners (CHP) – Mobile Acute Care division seeks an Advanced Practice Provider (FNP) to deliver in-home acute care, transitional care, and nurse-supported telehealth across the Greater Houston area. APPs operate with autonomy, manage visits using route optimization, and collaborate with physicians, nurses, and care coordinators.
The role requires active TX FNP licensure, MSN, prescriptive authority, BLS, and willingness to travel extensively.
Qualifications
- Active and unrestricted Texas licensure as a Family Nurse Practitioner (FNP).
- Master of Science in Nursing (MSN) from an accredited institution.
- Active Texas prescriptive authority.
- Current BLS certification.
- Valid driver's license and ability to travel throughout the Greater Houston area.
Responsibilities
- Perform comprehensive in-home histories and exams across acute presentations.
- Develop differential diagnoses and evidence-based treatment plans.
- Administer IV fluids and medications as indicated.
- Perform and interpret CLIA-waived point-of-care testing.
- Conduct in-home procedures (wound care, laceration repair, splinting, catheterization).
- Identify clinical deterioration and escalate when needed.
- Coordinate with interdisciplinary team and community partners.
