Job summary

Connected Healthcare Partners (CHP) is expanding its Mobile Acute Care division in the Greater Houston area. The Advanced Practice Provider (APP) will perform in-home acute care, post-discharge transitions, and nurse-supported telehealth consultations with autonomy and a collaborative interdisciplinary team.

Ideal candidates hold an FNP license in Texas, an MSN, prescriptive authority, and BLS; two-plus years in high-acuity settings are preferred.

Qualifications

• Active and unrestricted Texas FNP licensure required.
• MSN from an accredited institution.
• Active Texas prescriptive authority and BLS certification.
• Ability to travel across the Greater Houston area.

Responsibilities

• Deliver comprehensive in-home acute care and post-discharge transitional care.
• Manage complex chronic diseases with individualized care plans.
• Conduct post-discharge visits and medication reconciliation.
• Provide telehealth support and remote patient assessment.
• Collaborate with physicians, home health, and care coordinators.
• Complete documentation and ensure HIPAA and billing compliance.
