Qualificações

• Bachelor's Degree in mental health or related field required.
• Current Indiana Driver's License required.
• Two years of social service experience preferred.
• Ability to collaborate in a multidisciplinary team.
• Strong communication and de-escalation skills.

Responsabilidades

• Monitor and provide care coordination and life skills training in collaboration with the care team.
• Collaborate with internal and external stakeholders (e.g., Dept. of Child Services, juvenile probation).
• Document services in Electronic Medical Record (EMR) per policies.
• Transport clients in personal vehicles when necessary.
• Assist with data gathering for psychosocial assessments.
