Community Health Network is hiring a Behavioral Support Clinician (BSC) to provide care coordination and life skills development for youth with mental health and/or substance use disorders under the IMPACT program at Lutherwood. The work is delivered by a trauma-informed, strengths-based, multidisciplinary team to support residential services.

Applicants should collaborate with others, have excellent communication skills, and maintain a positive attitude toward problem-solving.

Qualifications

• Collaborate with a multidisciplinary care team to support youth with mental health/substance use disorders.
• Excellent communication skills and a positive attitude toward problem-solving are essential.
• De-escalation of clients displaying aggressive behaviors is required in this role.
• Documentation of services in the EMR follows policies and procedures.

Responsabilités

• Collaborates as part of a multidisciplinary care team.
• May assist with gathering data for comprehensive psychosocial assessments.
• Able to deescalate clients displaying aggressive behaviors.
• Documents services provided in EMR based on policies and procedures.
• When necessary, transports clients in personal vehicles.
