Job summary

Oak Street Health in Georgia is seeking a Nurse Practitioner to provide effective, equity-based primary care to adults on Medicare in outpatient clinics. You will practice on a collaborative care team with physicians, PAs, medical assistants, scribes, RNs and care managers to deliver longitudinal care and build strong patient relationships.

Responsibilities include wellness visits, care coordination, population health leadership, and education of patients about conditions and treatments.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in Family Nurse Practitioner; Adult-Gerontology Primary Care NP; Adult Nurse Practitioner; Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership, in coordination with the Care Team.
