Job summary

Partnership for Community Supports in Feasterville-Trevose, PA seeks Intellectual Disabilities Supports Coordinators to provide supports coordination for individuals with intellectual disabilities and autism, over the phone and in-person, including home and program visits.

You will develop person-centered plans, locate, coordinate and monitor services, and ensure health and safety for those on your caseload. Paid training, benefits, and a productivity award program are included.

Qualifications

• Bachelor’s degree with 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social science, or equivalent combination including two years’ experience as a County Social Service Aide 3 with related coursework.
• Valid driver’s license and use of personal automobile for home visits; mileage reimbursement.
• Excellent computer, verbal and written communication and organizational skills.
• Some experience with disabilities is preferred; bilingual ability is a plus; access to secure home WiFi is preferred.

Responsibilities

• Develop and enter Individual Support Plans into the Home and Community Services Information System within required time frames, involving the individual and team.
• Locate, coordinate and monitor services and family-driven supports for individuals with intellectual disabilities or autism.
• Monitor health and safety for individuals on assigned caseload.
• Facilitate team meetings with individuals, families and providers of service.
• Connect individuals with community resources and specialized services.
