Qualifications

• At least one year’s experience working with individuals with disabilities.

Responsibilities

• Develop and maintain the individual support plan using a person-centered approach.
• Coordinate community living supports across residential, vocational, educational and social services.
• Provide resources and support to families; evaluate progress toward goals.
• Respond to incidents to protect wellbeing, health, safety, and rights.
