The Commonwealth of Pennsylvania is seeking an Intellectual Disabilities Service Coordinator (County Caseworker 2) to help individuals and families identify strengths, address needs, and access supports. You will guide people toward greater independence within a collaborative state system.

The role emphasizes teamwork, problem solving, and steady encouragement as you coordinate services, monitor outcomes, and work with families, providers, and staff to maintain effective supports.

Qualifications

• Six months of experience as a County Caseworker 1; or approved internship program.
• Bachelor's degree with a social welfare major.
• Bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency.
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
• Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Responsibilities

• Needs Assessment: Review individual strengths and challenges to support planning decisions
• Plan Development: Collaborate with teams to create individualized support plans
• Service Coordination: Locate, arrange, and monitor services that match individual needs
• Eligibility Certification: Verify qualification for ID and autism services under regulations
• Ongoing Monitoring: Track service quality and make adjustments when needed
• Team Collaboration: Work with families, providers, and staff to maintain effective supports
