Job summary

Pathways Forward is seeking dedicated Supports Coordinators for the Intellectual and Developmental Disability Program in Harrisburg, PA. This full-time, day-shift role offers Monday–Friday schedules with no weekends.

You will create person-centered plans, connect supports, and monitor services to meet individual needs, with a focus on growth and a comprehensive benefits package. Relocation to Harrisburg, PA is required before starting.

Qualifications

• Bachelor's degree with ≥12 credits in sociology, social work, psychology, gerontology, criminal justice, or related social science.
• Associate degree with ≥12 credits in the above fields and two years of relevant experience.
• Clearances: PA Child Abuse, PA Criminal History, and FBI.
• Valid driver's license and reliable vehicle.

Responsibilities

• Create person-centered support plans with individuals, families, and support teams.
• Refer community and system supports to meet individual needs.
• Coordinate and monitor delivery of supports per plans.
• Ensure services meet type, duration, and frequency needs.
• Monitor health and wellbeing through regular contact with participants.
• Identify unmet needs and document for future planning and funding opportunities.
• Inform individuals of their rights and document via statewide system.
• Participate in ongoing training to enhance your skills.

What You'll Do:

As a Supports Coordinator, you will:

• Facilitate the creation of person-centered support plans with individuals, families, and support teams.
• Identify and refer community and system supports and services to meet individual needs.
• Coordinate and monitor the delivery of supports and services as outlined in support plans.
• Ensure the quality, type, duration, and frequency of services meet individual needs.
• Monitor health and wellbeing of participants through regular contact with individuals, families, and service providers.
• Identify unmet needs and document them for future planning and funding opportunities.
• Inform individuals of their rights and maintain comprehensive documentation through a statewide web-based system.
• Participate in ongoing training to enhance your skills and knowledge.
