Job summary

Delta Community Supports in Langhorne, PA seeks a dedicated coordinator to support participants in Intellectual Disability/DDD programs. The role focuses on ISP development, integration with the planning team, and ensuring services align with regulatory requirements.

The position reports to the Regional Director and requires managing documentation, annual assessments, and collaboration with family and providers to promote independence and community integration.

Qualifications

• Bachelor’s degree and 2 years’ experience supervising disability services.
• Associate degree with 4 years’ supervisory experience acceptable; multiple pathways.
• High School Diploma plus 6 years’ relevant community-based supervision experience acceptable.

Responsibilities

• Coordinate ISP development and annual ISP meetings with the team.
• Monitor progress and document outcomes for up to 30 individuals.
• Complete regulatory paperwork and annual assessments within required timelines.
• Supervise Direct Support Staff and provide training on ISP/ICP implementation.
• Ensure regulatory compliance with state, county, and Delta policies and procedures.
• Collaborate with Support Coordinators and planning team to meet service goals.
• Act as primary contact for DDD activities and communications.
