Job summary

Evergreen Treatment Services seeks a SUDP/SUDPT Assessor to coordinate primary care for assigned patients within Seattle Clinic and DESC Orca Center, conducting SUD assessments in varied settings. The role supports community partners to deliver onsite assessments where needed and contributes to treatment planning and care coordination.

You will perform initial assessments, develop treatment plans, and collaborate with clinicians to ensure continuity of care.

Qualifications

• SUDP license or trainee as defined by WAC 246-811-030 and related DOH requirements.
• Academic training in social services and substance use disorder treatment per DOH qualifications.
• Work experience in social services and substance use disorder treatment as per SUDP/T qualifications.
• Confidentiality, ethics, and compliance with 42 CFR Part 2 regulations.

Responsibilities

• Perform Substance Use Disorder assessments in various settings and develop treatment plans.
• Respond to referrals to perform assessments, provide care coordination and treatment recommendations.
• Develop relationships with community resources to support treatment and safety planning.
• Provide and connect patients with program and agency resources.
• Maintain agency policies and timely documentation of patient interactions.
• Coordinate patient treatment with other staff departments and participate in Treatment Team meetings.
