Aviator Home Health in Denton, TX is seeking a licensed Speech Language Pathologist to provide in-home therapy for speech, language, swallowing and cognitive disorders. Qualifications

• Two years experience in an acute care or rehabilitation setting; home health preferred.
• Texas license required to practice.
• Master’s or Doctorate in Speech-Language Pathology from an accredited program.

Responsabilités

• Perform initial and ongoing clinical assessments to identify patient needs and candidacy for home care.
• Develop and revise Plan of Care in consultation with physicians and care team.
• Provide therapy for speech, language, cognitive and swallowing disorders with appropriate tests and measurements.
• Evaluate outcomes and assist with discharge planning.
• Maintain infection control practices and standard precautions during patient care.
• Supervise speech language assistant and HHA as appropriate.
• Complete documentation in a timely manner.
• Meet continuing education requirements of the agency/licensing board.
