Job summary

Aviator Home Health is seeking a Speech Language Pathologist to provide therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders in home settings.

The role includes performing initial and ongoing assessments, developing plans of care with physicians, treating disorders, documenting outcomes, and supervising support staff while ensuring infection control and agency policies are followed.

Qualifications

• Master’s Degree or Doctorate in Speech-Language Pathology from an accredited college or university.
• Current Texas State License in Speech-Language Pathology and a valid Texas Driver's License.
• Two years of experience in an acute care or rehabilitation setting; home health preferred.

Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs and functioning.
• Develop and revise Plan of Care in consultation with physician and care team.
• Provide therapeutic treatments for speech, language, cognitive and swallowing disorders.
• Evaluate outcomes and plan discharge; complete timely documentation.
• Supervise speech language assistants and HHA as appropriate.
• Incorporate infection control concepts and agency policies in care delivery.
• Communicate effectively with families and agency personnel.
