Job summary

Aviator Home Health is seeking a Speech Language Pathologist to deliver therapeutic techniques for rehabilitation of speech, language, hearing, oral motor, swallowing and cognitive disorders in a home health setting.

Responsibilities include performing initial and ongoing assessments, developing plans of care with the physician, and supervising aides as appropriate; accurate documentation and CE participation are required.

Qualifications

• Master’s or Doctorate in Speech-Language Pathology required.
• Current Texas State License in Speech-Language Pathology.
• Texas Driver’s License required.
• Two years experience in acute care/rehabilitation; home health preferred.

Responsibilities

• Perform initial and ongoing clinical assessments, including OASIS timing.
• Develop and revise Plan of Care in consultation with physician and care team.
• Provide therapies for speech, language, cognitive and swallowing disorders.
• Evaluate outcomes and plan discharge.
• Supervise SLP assistant and HHA as appropriate.
• Complete timely documentation and meet CE requirements.
