Job summary

Aviator Home Health in Texas seeks a Speech-Language Pathologist to provide therapeutic services for speech, language, swallowing, oral motor and cognitive disorders in home care settings.

You will perform initial and ongoing assessments, develop and revise plans of care with the physician and team, and ensure evidence-based care is delivered. Supervision of SLAs and documentation are key duties.

Qualifications

• Master’s or Doctorate in Speech-Language Pathology from an accredited program.
• Current Texas State License in Speech-Language Pathology.
• Two years of acute care or rehabilitation experience preferred, home health is a plus.

Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs and functioning.
• Develop and revise Plan of Care in collaboration with physicians and care team.
• Provide therapeutic treatments for speech, language, cognitive and swallowing disorders.
• Evaluate outcomes and participate in discharge planning.
• Apply infection control and standard precautions in all patient activities.
• Supervise speech-language assistant and HHA as appropriate.
• Complete timely documentation and meet continuing education requirements.
