Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs and determine home care suitability.
• Develop and revise Plan of Care in consultation with physicians and care team.
• Provide therapeutic treatments for speech, language, cognitive and swallowing disorders with appropriate tests.
• Evaluate outcomes and plan for discharge.
• Complete timely documentation and maintain accurate records.
• Supervise speech language assistant and HHA as appropriate.
• Meet continuing education requirements and participate in in-service programs.
• Communicate with family and agency personnel to coordinate care.
