Job summary

Aviator Home Health is seeking a Speech Language Pathologist to deliver evidence-based therapies for speech, language, swallowing, and cognitive disorders in home health patients. You will conduct assessments, develop plans of care with the care team, and provide hands-on therapy with thorough documentation.

Texas licensure and a current Texas driver’s license are required, with at least two years in acute or rehab settings; home health experience is preferred.

Qualifications

• Master’s or doctoral degree in Speech-Language Pathology from an accredited program.
• Current Texas State License in Speech-Language Pathology and valid Texas Driver’s License.
• Two years of experience in acute care or rehabilitation; home health preferred.
• Ability to develop and revise plans of care with physicians and care teams.
• Experience providing services for speech, language, swallowing, and cognitive disorders.

Responsibilities

• Perform initial and ongoing clinical assessments to identify patient needs and function.
• Develop and revise the Plan of Care in consultation with the physician and team.
• Provide therapeutic treatments for speech, language, cognitive and swallowing disorders.
• Evaluate outcomes and participate in discharge planning.
• Apply infection control concepts in patient care activities.
• Accept assignments within education and competence level.
• Supervise Speech Language Assistant and HHA as appropriate.
• Complete timely documentation and participate in ongoing education.
