Job summary

Creative Solutions in Healthcare - East is seeking a full-time Speech Language Pathologist to join our in-house rehab team serving residents with person-centered care. This role supports positive outcomes through skilled therapy and individualized treatment plans.

The ideal candidate holds a valid Texas SLP license, SNF/long-term care experience, and is committed to education for residents and caregivers while delivering evidence-based therapy and precise documentation.

Qualifications

• Active Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies and boards.
• SNF/Long term care experience preferred.
• Knowledge of current treatment practices.
• Familiarity with Net Health system preferred.
• Ability to work with residents with varying personalities.

Responsibilities

• Evaluate residents for swallowing or cognitive deficits.
• Assist residents to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled speech therapy assessments and develop treatment plans.
• Provide AAC systems for severe expressive/language disorders.
• Train clients, families, caregivers, and professionals.
• Coordinate with professionals for swallow procedures as ordered.
• Demonstrate competence with current Speech Therapy standards.
• Communicate changes in residents' condition.
• Maintain accurate billing and clinical documentation.
• Respond to family requests timely.
• Coordinate with Restorative Nursing and other staff.
• Ensure resident safety and confidentiality.
• Comply with legal and professional requirements.
