Creative Solutions in Healthcare, Inc. is seeking a qualified Speech-Language Pathologist to evaluate residents, develop and implement treatment plans, and support communication and swallowing goals in a SNF setting in Texas.

Qualifikationen

• Active Texas license in Speech-Language Pathology.
• Experience in SNF/Long-term care preferred.
• Knowledge of current treatment practices.
• Familiarity with Net Health Electronic Documentation System.

Aufgaben

• Evaluate residents' swallowing or cognitive deficits and plan care.
• Develop and implement individualized treatment plans to increase independence.
• Provide AAC systems for individuals with severe expressive/language disorders.
• Educate residents, caregivers and family members; provide training to professionals as needed.
• Coordinate with physicians and other professionals for diagnostic procedures as ordered.
• Maintain accurate billing records and clinical documentation; ensure confidentiality and compliance.
