Acadia Healthcare is seeking a qualified social worker in Fort Wayne, IN to plan and coordinate therapeutic services for patients with behavioral health needs. You will conduct psychosocial assessments, develop treatment plans, and oversee discharge planning within a multidisciplinary team.

Requires a Master's in Social Work with at least three years of related experience; licensure such as LMSW/LCSW/LSW is often required.

Competenze

• Master's degree in Social Work from an accredited college or university.
• Three or more years' related experience working with comparable patient populations.
• State licensure as LMSW / LCSW / LSW may be required by the facility.

• Conduct psychosocial assessments and develop treatment plans.
