CHS Career Site is seeking a Licensed Master Social Worker (LMSW) for a PRN Day Shift role at Lutheran Hospital in Fort Wayne, IN. You will provide psychosocial assessments, counseling, and care coordination to support patient transitions and discharge planning.

The role requires an MSW (or Psychology) degree, LMSW licensure, and 2–4 years in healthcare, with strong collaboration across the care team and strong documentation in EMR systems.

Formación

• MSW or Psychology degree required.
• 2-4 years healthcare social work experience preferred.
• LMSW license required; BCLS certification required within 7 days of employment.

Responsabilidades

• Conduct psychosocial assessments across the lifespan.
• Coordinate discharge planning and community referrals.
• Provide emotional support and counseling to patients and families.
• Document timely and accurate care plans in EMR.
