Qualifications

Responsibilities

• Conduct and document psychosocial assessments.
• Help clients access medical and social services via referrals.
• Promote self-sufficiency and coordinate service delivery.
• Assist with patient care in clinic or home visits.
• Liaise with internal departments and external agencies to ensure continuity of care.
• Educate patients on disease transmission, medications, and healthy lifestyles.
• Ensure accurate, timely care documentation per guidelines.
• Identify and apply for financial assistance and expanded social services.
• Perform other duties as assigned.
