Qualifications

Responsibilities

• Provides psychosocial assessments and documents findings.
• Coordinates with multidisciplinary team for discharge planning and care continuity.
• Assists patients in accessing medical and social services through direct support or referrals.
• Empowers clients toward self-sufficiency and coordinates service delivery to prevent duplication.
• Assists with patient care in clinical or home visit settings.
• Liaises with internal departments and external agencies to ensure continuity of care and resources.
