Intermountain Health seeks a Social Work Care Manager to deliver patient-centered care management within our Healthcare & Medical Services team. You’ll complete psychosocial assessments, develop individualized care and discharge plans, coordinate community and behavioral health resources, and support patients and families through transitions of care.

Qualifikationen

• Current state Social Work license (e.g., LBSW, LMSW, LCSW or equivalent)
• Bachelor’s or Master’s degree in Social Work from an accredited program
• Experience in healthcare or hospital-based social work preferred
• Knowledge of community and behavioral health resources
• Bilingual skills preferred

Aufgaben

• Conduct comprehensive psychosocial assessments for patients and families
• Develop and implement individualized care and discharge plans
• Coordinate community, behavioral health, and social service resources
• Collaborate with an interdisciplinary team to support safe care transitions
• Provide crisis intervention, counseling, and patient/family education
• Address social determinants of health impacting care and outcomes
• Advocate for patient needs and remove barriers to access and continuity of care
• Document assessments, plans, and outcomes in the electronic health record
• Participate in quality improvement and care management initiatives
• Maintain compliance with regulatory, ethical, and organizational standards
