Qualifications

Responsibilities

• Assist in planning, organization, implementation, and evaluation of the facility’s Social Services Department.
• Ensure social services comply with federal, state, and local guidelines and Harborview values.
• Develop and monitor individualized care plans for residents' social and emotional needs.
• Provide ongoing support to residents and families during admissions, transitions, and care conferences.
• Recommend equipment, services, or referrals for outpatient and home health support.
• Participate in policy development and propose improvements to care delivery.
• Organize and participate in in-service training for the social services team and staff.
• Stay up-to-date on regulatory changes and long-term care social work standards.
• Advocate for residents and liaise among staff, families, and external agencies.
• Collaborate with interdisciplinary care teams for holistic, person-centered care.
• Duties as assigned.
