Job summary

Harborview Health Systems is seeking a Social Services Director to lead the facility's social services department, ensuring compliance with federal and state regulations while guiding care planning and family support.

You will collaborate with interdisciplinary teams to deliver person-centered, holistic care and advocate for residents throughout admissions and transitions. A BSW or related degree and relevant licensure are required.

Qualifications

• Bachelor’s degree in Social Work (BSW) or related human services field such as sociology, psychology, special education, or rehabilitation counseling.
• Minimum of 1 year supervised social work experience in a healthcare setting.
• Knowledge of federal and state long-term care regulations.
• Strong interpersonal, communication, and problem-solving skills.
• Resident-first mindset with dignity, empathy, and respect; professionalism in sensitive situations.

Responsibilities

• Assist in the planning, organization, implementation, and evaluation of the facility’s Social Services Department.
• Ensure social services comply with federal, state, and local guidelines and Harborview's policies.
• Develop and monitor individualized care plans for residents addressing social and emotional needs.
• Provide ongoing support to residents and families during admissions, transitions, and care conferences.
• Recommend equipment, services, or referrals for outpatient and home health support.
• Participate in policy development and improvements to care delivery.
• Organize and participate in in-service trainings for social services and staff.
• Stay updated on regulatory changes and professional standards in long-term care social work.
• Advocate for residents and liaise among staff, families, and external agencies.
• Collaborate with interdisciplinary teams for holistic, person-centered care.

What You Will Do

• Organize and participate in in-service training sessions for the social services team and other staff
• Stay up-to-date on regulatory changes and professional standards related to long-term care social work
• Advocate for residents and serve as a liaison among staff, families, and external agencies
• Collaborate with interdisciplinary care teams to ensure holistic, person-centered care
• Duties as assigned

What You Must Have

• Bachelor’s degree in Social Work (BSW), or in a human services field such as sociology, psychology, special education, or rehabilitation counseling
• Minimum of1 year supervised social work experiencein a healthcare setting working directly with individuals
• Knowledge of federal and state long-term care regulations
• Strong interpersonal, communication, and problem-solving skills
