Job summary
Saber Health Group is seeking a Social Services professional to lead the Social Services Department in a skilled nursing facility, ensuring compassionate, compliant, and person-centered care. You will design, implement, and evaluate social programs to support residents’ emotional and social needs, in line with regulations and facility policies.

Qualifications
- Bachelor’s degree in social work or related field as required by state/federal rules.
- At least 1 year of supervised social work experience in healthcare settings.
- Knowledge of regulations governing social services in nursing facilities.
- Basic computer skills and ability to navigate electronic health records.
Responsibilities
- Lead planning and oversight of the Social Services Department in a skilled nursing facility.
- Ensure resident psychosocial well-being and compliance with regulations.
- Coordinate etc planning and evaluation of resident social services programs.
