Visalia Post Acute in Visalia, CA is seeking a compassionate, organized and experienced Full-Time Social Services Director to support residents social, emotional, and psychosocial well-being. The role collaborates with residents, families, nursing staff, therapists, and community agencies to advocate for residents in sensitive situations.

A minimum of a bachelor degree is required; a Master of Social Work is preferred.

Qualificações

• Bachelor degree in Social Work, Human Services, Psychology, Sociology, or a closely related field required.
• At least two years of supervised social-work experience in a healthcare setting working directly with individuals required.
• Master of Social Work degree preferred.

Responsabilidades

• Assess residents social, emotional, psychological, and family needs.
• Develop goals and action plans with residents and families.
• Provide medically related social services to support physical, mental, and psychosocial well-being.
• Assist with social-services care plans and discharge planning.
• Coordinate referrals to community resources and arrange outpatient services.
