Job summary

Ahava Therapy Services in Henderson, TN is seeking a licensed Speech-Language Pathologist to join a collaborative rehab team at Henderson Health & Rehab, a 137-bed facility providing short- and long-term care and outpatient services.

You will evaluate and treat residents under Medicare/Medicaid guidelines, develop individualized plans, document progress, and collaborate with nursing staff to maximize residents' independence. Ongoing CEUs and professional standards apply.

Qualifications

• Master's Degree in Speech-Language Pathology.
• CCC-SLP certification.
• Active SLP license in the state of practice.
• Current BLS/CPR certification.
• Excellent observational, verbal, and written communication skills.

Responsibilities

• Evaluate and treat residents in accordance with Medicare, Medicaid, and payer guidelines.
• Conduct comprehensive assessments for swallowing, cognitive-communication, and language deficits.
• Develop and implement individualized treatment plans with functional goals.
• Document evaluations, daily treatment, progress notes, and discharge summaries accurately.
• Collaborate with nursing staff and interdisciplinary team to maximize function.
• Participate in care conferences, facility meetings, and quality improvement activities.
• Discharge planning and community resource referrals as needed.
• Stay current with continuing education and participate in in-service education.
