Job summary

Ahava Therapy Services in Henderson, TN seeks a Speech-Language Pathologist (SLP) to deliver evidence-based therapy across care settings. You will evaluate swallowing, cognition, and language deficits, and implement individualized plans to maximize patient potential.

You will collaborate with nursing and therapy teams, contribute to quality-improvement activities, and guide families and residents through discharge planning, while maintaining up-to-date clinical knowledge and professional

Qualifications

• Master's degree required in Speech-Language Pathology.
• CCC-SLP certification/credential required.
• Active state SLP license required.
• Current BLS/ CPR certification.
• Strong observation, verbal, and written communication skills.

Responsibilities

• Evaluate and treat residents per Medicare, Medicaid, and payer guidelines.
• Conduct comprehensive assessments for swallowing, cognitive-communication, and language deficits.
• Develop and implement individualized treatment plans with functional goals.
• Document evaluations, daily treatment, progress notes, and discharge summaries promptly.
• Revise treatment plans based on patient progress.
• Educate residents, families and caregivers on techniques and carryover strategies.
• Collaborate with nursing staff, interdisciplinary teams, and families to maximize function.
• Participate in care conferences, facility meetings, and quality improvement activities.
• Assist with discharge planning and community resource referrals.
• Stay clinically current through continuing education and in-service learning.
• Adhere to professional, regulatory, and facility standards.
• Maintain productivity and strong teamwork and clinical judgment.
