Job summary

Ahava Therapy Services is seeking a Full-Time Speech-Language Pathologist for Cordova Wellness & Rehab Center, near Memphis, TN. The role focuses on evaluating and treating residents with Medicare/Medicaid guidelines, with collaborative, patient-centered care.

The position requires a Master’s in Speech-Language Pathology, an active SLP license in the practice state, and BLS/CPR certification, along with strong communication and teamwork abilities. CFYs accepted.

Qualifications

• Master's Degree in Speech-Language Pathology.
• CFY accepted.
• Active SLP license in the state of practice.
• Current BLS/CPR certification.
• Strong observational, verbal, and written communication skills.

Responsibilities

• Evaluate and treat residents per payer guidelines.
• Conduct comprehensive swallowing, cognitive-communication, and language assessments.
• Develop and implement individualized treatment plans with functional goals.
• Document evaluations, daily treatment, progress notes, and discharge summaries.
• Educate residents, families, and caregivers on therapy techniques.
• Collaborate with nursing staff and interdisciplinary teams.
• Participate in care conferences and quality improvement activities.
• Assist with discharge planning and community resource referrals.
• Maintain up-to-date clinical knowledge and continue education.
• Adhere to professional and facility standards.
• Maintain productivity and strong teamwork.

Ahava Therapy Services is hiring a Full-Time SLP for Cordova Wellness & Rehab Center, a 242-bed skilled nursing facility offering short-term rehab, long-term care, and outpatient therapy services.

Responsibilities

• Evaluate and treat residents in accordance with Medicare, Medicaid, and commercial payer guidelines.
• Conduct comprehensive assessments to determine swallowing, cognitive-communication, and language deficits.
• Develop and implement individualized treatment plans with functional, achievable goals.
• Document evaluations, daily treatment, progress notes, and discharge summaries accurately and promptly.
• Revise treatment plans based on patient performance and progress.
• Educate residents, families, and caregivers on therapeutic techniques and carryover strategies.
• Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional abilities.
• Participate in care conferences, facility meetings, and quality improvement activities.
• Maintain up-to-date knowledge of community resources and assist with discharge planning as needed.
• Stay clinically current through continuing education and contribute to in-service education.
• Adhere to professional, ethical, regulatory, and facility standards.
• Maintain expected productivity levels and demonstrate strong teamwork, communication, and clinical judgment.
