Job summary

Ahava Therapy Services is seeking a full-time Speech-Language Pathologist (SLP) to join our in-house rehabilitation team at Wexford House, a 173-bed skilled nursing facility in Kingsport, TN. You will evaluate and treat residents, develop individualized plans, and collaborate with an interdisciplinary team to maximize functional abilities.

The position requires a Master's in Speech-Language Pathology, active SLP licensure in the state, BLS/CPR, and CFY eligibility.

Qualifications

  • Master’s Degree in Speech-Language Pathology.
  • CFY’s allowed to apply.
  • Active SLP license in the state of practice.
  • Current BLS/CPR certification.
  • Strong observational, verbal, and written communication skills.

Responsibilities

  • Evaluate and treat residents in accordance with Medicare, Medicaid, and commercial payer guidelines.
  • Conduct comprehensive assessments to determine swallowing, cognitive-communication, and language deficits.
  • Develop and implement individualized treatment plans with functional, achievable goals.
  • Document evaluations, daily treatment, progress notes, and discharge summaries accurately and promptly.
  • Revise treatment plans based on patient performance and progress.
  • Educate residents, families, and caregivers on therapeutic techniques and carryover strategies.
  • Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional abilities.
  • Participate in care conferences, facility meetings, and quality improvement activities.
  • Maintain up-to-date knowledge of community resources and assist with discharge planning as needed.
  • Stay clinically current through continuing education and contribute to in-service education.
  • Adhere to professional, ethical, regulatory, and facility standards.
  • Maintain expected productivity levels and demonstrate strong teamwork, communication, and clinical judgment.
