Job summary

AMEDISYS HOME HEALTH is seeking an RN Case Manager for full-time daytime hours to support home health patients. You will assess, plan, and coordinate care to help patients achieve optimal outcomes and independence, working with a multidisciplinary team and supervising field staff.

Responsibilities include on-going patient assessment, referrals, documentation, and staff education. Candidates must hold an active RN license and CPR certification, with a valid driver’s license and reliable

Qualifications

• One (1+) year of clinical experience as an RN.
• Current RN license for the assigned state(s).
• Current CPR certification is required.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as needed and document rationale.
• Promote patient health and independence through patient teaching and rehab measures.
• Supervise LPNs and HHAs.
• Complete timely and accurate documentation.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education and on-call coverage as assigned.
• Participate in clinical development and continuing education programs.
