Job summary

Amedisys is seeking a part‑time RN Case Manager to perform patient assessments, develop and implement care plans, and collaborate with the care team. You will educate patients to promote self-care and supervise home health staff as assigned.

The role requires an active RN license, CPR certification, and 1+ year of clinical RN experience. Flexible days and mileage reimbursement are included as benefits.

Qualifications

• Minimum 1+ year of clinical RN experience.
• Active RN license for the state(s) of work.
• Current CPR certification.

Responsibilities

• Perform patient assessments and collaborate with care team on care plans.
• Refer to other disciplines as needed and document rationale.
• Educate patients to promote health and independence and supervise staff as needed.
• Complete timely and accurate documentation and report progress to management.
• Participate in on-call responsibilities and ongoing clinical development.
