Job summary

Amedisys Home Health is seeking a Registered Nurse Case Manager to provide high-quality patient care in a home health setting. You will assess patients, develop and implement plans of care, and coordinate with the care team to promote recovery at home.

You will supervise LPNs and HHAs, complete timely documentation, and communicate patient progress to clinical leadership. Opportunities for staff education, on-call shifts, and ongoing professional development are available.

Qualifications

• 1+ year of clinical RN experience (or approved by VP Clinical).
• Current RN license for the state(s) assigned.
• Current CPR certification.

Responsibilities

• Assess patients and collaborate with the care team to create and implement a plan of care.
• Refer to other disciplines as needed and document rationale.
