Job summary

Amedisys Home Health is seeking a Registered Nurse Case Manager for full-time day shifts. You will assess patients, collaborate with the care team, and implement a comprehensive plan of care in the home setting.

Responsibilities include making referrals, patient education, supervising HHAs and LPNs, and on-call coverage. Qualifications include 1+ year RN experience, current state RN license, CPR certification, and a valid driver’s license with reliable transportation and insurance.

Qualifications

• 1+ year of clinical RN experience; if less, VP Clinical approval is required.
• Current RN license for the assigned state(s).
• Current CPR certification required.
• Valid driver’s license with reliable transportation and liability insurance.
• RN intern program if less than 6 months RN experience.

Responsibilities

• Performs patient assessments and collaborates with care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale for not referring.
• Promotes patient health and independence through teaching and rehabilitative measures; supports self-care learning.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress regularly to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services as assigned.
• Participates in clinical development and continuing education.
