Job summary

Volunteers of America Colorado is seeking a Service Coordinator for residents in an Affordable Housing portfolio. You will conduct needs-based assessments, provide case management, and connect residents to income resources and public benefits.

The role emphasizes trauma-informed care, harm reduction, and aging-in-place approaches. Work closely with the Community Administrator, maintain records, and develop programs to foster community wellbeing and independence among seniors and people with

Qualifications

• Bachelor's degree in human services, social work, or a closely related field or related experience.
• At minimum, one year of working with senior citizens and/or people with disabilities.
• Willing to obtain first aid/CPR/AED certifications.
• Criminal and civil background checks within 30 days of hire.
• Excellent interpersonal and organizational skills.
• Proficient in Windows and databases.

Responsibilities

• Ensure residents' voices are heard and participate in programming.
• Facilitate referrals from Community Administrator, families, and residents.
• Assess resident needs via surveys, visits, groups, etc.
• Provide services per trauma-informed care and equity lens.
• Inform residents about services and program providers with options.
• Encourage residents to arrange required services to maintain independence.
• Coordinate and monitor providers to ensure quality of care and cost effectiveness.
• Maintain up-to-date records on residents and providers.
