Job summary

TASC is seeking a full-time Case Manager to serve clients statewide in Illinois, starting at a salary range of 44,676 to 47,650 USD, depending on experience. The role includes addiction intervention, AODA counseling, and recovery support for pregnant and postpartum mothers and families.

Qualifications

• High school diploma or GED required.
• CADC certification within two years of employment may be required.
• Valid driver’s license.
• Must pass an IDOC background check.
• Proficiency with Microsoft Office suite.
• Experience facilitating groups.
• Knowledge of ASAM DSM-V placement criteria for substance use disorders.

Responsibilities

• Assist clients in achieving service plan goals through coordination with behavioral health, public agencies and community resources.
• Complete intake packets for program enrollment and participation.
• Provide referrals and crisis intervention for individuals upon release from prison.
• Collaborate with internal and external partners and participate in staff meetings and supervised sessions.
• Maintain confidential information and ensure progress reports are documented.

Division Overview

The Community Reentry CR is a statewide network of clinical reentry management services with the overall purpose of increasing opportunities for successful reentry outcomes CR provides specialized case management services to help people prepare to return to their families and communities after incarceration CR works both inside prisons and in communities to provide reentry support including but not limited to behavioral health referrals public benefits enrollment finding employment and housing resources and obtaining state IDs and vital records

Position Summary

We are currently looking for full time Case Manager The position is statewide and may serve clients in any county throughout Illinois Starting at 44676 47650; contingent upon experience education etc This position will provide addiction intervention AODA counseling and recovery support services to pregnant and post partum mothers and their families This position is responsible for bringing services agencies resources and other people together to assist clients in achieving established goals This position may also work with high risk caseloads co occurring clients families and children

Key Responsibilities

• Assist individuals in achieving service plan goals through direct services and coordination of behavior health public agencies and community resources
• Assess evaluate facilitate and monitor individuals progress with contractual benchmarks
• Complete intake packets for program enrollment and participation
• Provides referrals and crisis intervention for the individuals identified needs upon release from prison
• Interview individuals review records and confer with other professionals to identify reentry needs
• Collaborate with internal and external partners participate in staff meetings and monthly supervisions and participate in integrated staffing with individuals receiving TASC services
• Identify and refer individuals to initial treatment and behavioral health services
• Complete and submit internal and external program reports
• Complete and maintain all required individual and program related documentation
• Assist clients with the enrollment in public benefit systems and obtaining vital records
• Participate in community meetings trainings and projects including multi disciplinary staffing vocational and educational programming institutional counseling correctional industries employers and other meetings assigned
• Develops effective working relationships with the internal and external partners
• Participate in meetings to adjust update or revise individual service plan
• Obtain and clinically analyze progress reports from internal and external partners to ensureindividuals progress is on target with the benchmarks outlined in their service plan
• Ensure that confidential information relating to the organization its staff and individuals is kept confidential
• Be an excellent steward of TASC modeling the core values of the organization

Ideal Candidate

• High school diploma or GED equivalent
• Valid drivers license
• CADC certification within two years of employment with TASC may be required based on contractual obligations
• Must be able to pass an IDOC Background check
• Demonstrated proficiency in the Microsoft Office suite
• Demonstrated experience in group facilitation
• Knowledge of the current ASAM placement criteria DSM V as it relates to substance related disorders and Code of Ethics for substance abuse professionals or the ability to acquire the knowledge
• Comfort working inside a correctional setting and with persons convicted of serious violent offenses
• Flexibility in thinking and approaches to leading group discussions
• Good verbal written and interpersonal communication skills
• Good time management skills with a proven ability to meet deadlines
