Pinnacle Treatment Centers in California is seeking Counselors to support recovery journeys. The role involves conducting assessments, developing treatment plans, and coordinating with medical leaders.

Qualifications

• Provide professional counseling and referral services; address clinical needs and maintain referrals to external services.
• Conduct biopsychosocial assessments and ongoing evaluations to guide treatment plans.
• Utilize standardized screening tools (BAM-R, C-SSRS, GAD-7, PHQ-9) to assess patient status.

Responsibilities

• Provide orientation to new patients on program rules, policies, and patient rights.
• Conduct intakes, discharge planning, initial assessments and treatment planning within guidelines.
• Develop individualized treatment plans using SNAP to align with patient goals.
• Monitor changes that affect recovery and address relapse indicators early.
• Review physician notes and coordinate with Medical Directors or RCL as needed.
• Attend team meetings and complete required trainings.
