Bachelor’s degree in psychology, social work, or related field required.
At least two years of related mental health experience; jail diversion/OCR preferred.
Knowledge of Trauma-Informed Care Practices required.
Reliable transportation and valid driver’s license.

Responsibilities

Provide outpatient case management, skills training, medication training and psychosocial rehabilitation services.
Offer community-based, person-directed case management to access medical, social, vocational, financial, educational, legal and other services.
Initiate and document service plans to meet individual needs in the community, home, legal system, and school.
Collaborate with criminal justice agencies and community partners to increase referrals and resources.
Maintain timely documentation and strong boundaries to improve outcomes.
