Community Options, Inc. in San Antonio, TX is seeking an experienced Provider Service Coordinator to assess the needs of individuals with intellectual and developmental disabilities and identify appropriate support services.

The PSC develops ISP/PCP with an interdisciplinary team, advocates for clients, coordinates resources, and ensures documentation for eligibility. Requires HS diploma or GED; valid driver’s license; Bachelor's preferred.

Qualifications

• High school diploma or GED with two years of experience supporting individuals with developmental disabilities.
• Bachelor's degree preferred; may be required in some states.

Responsibilities

• Advocate for the needs and choices of individuals in our care.
• Assist with determining appropriate support services during placement and transition.
• Develop and update ISP/PCP with an interdisciplinary team to include concerns, observations, and behavioral developments.
• Develop and monitor training goals and objectives that align with the ISP/PCP.
• Communicate with families and guardians of individuals we support as needed.
• Identify, develop, and coordinate community resources and support.
• Submit required documentation to maintain eligibility for government assistance programs.
• Ensure program documentation and billable records are completed accurately and timely.
• May perform the role of direct support professional as necessary when staffing needs arise.
