Job summary

Community Options, Inc. seeks an experienced Provider Service Coordinator in Austin/Pflugerville, TX. The PSC assesses needs of individuals with intellectual and developmental disabilities and coordinates supports to promote development across physical, cognitive, emotional, and social skills.

The starting salary is $44,000 per year. The role requires advocacy, service planning, and collaboration with families, clinicians, and community programs. Equal opportunity employer.

Qualifications

• High school diploma or GED with two years of experience supporting individuals with developmental disabilities.
• Bachelor’s degree preferred; may be required in some states.
• Valid driver’s license with a satisfactory driving record.
• Team-oriented with leadership experience.
• Experience solving problems with multiple priorities.
• Ability to establish a supportive relationship with individuals receiving supports.

Responsibilities

• Advocate for the needs and choices of individuals in our care.
• Assist with determining appropriate support services during placement and transition.
• Develop and update ISP/PCP with an interdisciplinary team, including concerns and behavioral developments.
• Develop and monitor training goals aligned with the ISP/PCP.
• Communicate with families and guardians as needed.
• Identify and coordinate community resources and support.
• Submit documentation to maintain eligibility for government programs.
• Ensure program documentation and billable records are completed accurately and timely.
• Serve as an active member on each individual’s planning team.
• May perform the role of direct support professional as needed during staffing gaps.
• Cooperate with license and department staff during inspections or inquiries.

Education
