Job summary

Community Options, Inc. in Austin / Pflugerville, TX is seeking an experienced Provider Service Coordinator to assess needs and coordinate support services for individuals with developmental disabilities. You will advocate for clients and help develop ISP/PCP with an interdisciplinary team, promoting learning and life skills development.

Qualifications

• High school diploma or GED with two years of experience supporting individuals with developmental disabilities.
• Bachelor’s degree preferred; may be required in some states.
• Valid driver’s license with a satisfactory driving record.
• Team-oriented with demonstrated leadership experience.
• Experience solving problems across multiple priorities.
• Ability to establish a comfortable and supportive relationship with individuals receiving supports.
• Employment is contingent upon successful checks of criminal background, central registry, child abuse registry, and drug testing.

Responsibilities

• Advocate for the needs and choices of individuals in our care.
• Assist with determining appropriate support services during placement and transition.
• Develop and update ISP/PCP with an interdisciplinary team.
• Develop and monitor training goals aligned with the ISP/PCP.
• Communicate with families and guardians as needed.
• Identify, develop, and coordinate community resources and support.
• Submit required documentation to maintain eligibility for government programs.
• Ensure program documentation and billable records are completed accurately and timely.
• Serve as an active member on each individual’s planning team.
• May perform the role of direct support professional when staffing needs arise.
• Additional tasks may be assigned.
• Cooperate with license and department staff during inspections or investigations.
