Qualifications

• Must have a Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and facility procedures.
• Adhere to documentation and billing procedures for therapy services.
• Develop and implement treatment plans with short and long-term goals.
• Conduct regular screenings to determine intervention needs.
• Evaluate residents to gather data for treatment planning and communicate findings to residents, families and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Collaborate with the interdisciplinary team to discharge responsibilities effectively.
