Job summary

Evolve Therapy Services in Saegertown, PA, is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility on a PRN basis. You will provide PT services to residents, perform screenings, develop and adjust treatment plans, and collaborate with an interdisciplinary team.

The role requires a BS in Physical Therapy, an active PT license in state(s) of practice, and commitment to CMS documentation using NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to CMS documentation standards and CMS expectations.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate and plan resident therapy programs with short and long-term goals.
• Develop treatment plans and communicate findings to residents and families.
• Perform screenings to determine intervention needs and coordinate with interdisciplinary team.
• Complete compliant documentation using NetHealth EMR as required.
