Job summary

Evolve Therapy Services, LLC is seeking a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Pahokee, FL. The position offers full-time, part-time, or PRN schedules to support resident care in a collaborative setting.

Responsibilities include planning, implementing, and documenting therapy services, conducting screenings, and communicating with residents and the interdisciplinary team. A BS in Physical Therapy and an active license are required.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and CMS billing requirements.
• Evaluate and plan resident therapy programs with interdisciplinary input.
• Screen residents regularly to determine intervention needs.
• Screen residents referred for PT to determine need for intervention.
• Evaluate residents to collect data for treatment planning.
• Develop and communicate treatment plans with goals.
• Complete compliant documentation in NetHealth EMR.
