Job summary

Evolve Therapy Services, LLC is hiring a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Pahokee, FL. The PRN position supports residency care with direct supervision from a Physical Therapist and a focus on resident mobility and functional goals.

Responsibilities include screenings, treatment planning, and coordination with the therapy team. Requires an Associate's degree and an active PTA license in the state, with ongoing education to maintain licensure.

Qualifications

• Associate's degree required in physical therapy from an accredited program.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist in evaluations and in developing target goals and treatment plans.
• Conduct individual and group treatment focusing on strength, mobility, gait, range of motion, and modalities.
• Assist in selecting techniques and adapting activities based on resident progress.
• Monitor resident response and adjust as directed by the Physical Therapist.
• Assist in scheduling and coordinating resident therapy with the team.
• Participate in resident, family, and staff education.
• Ensure timely, compliant documentation in NetHealth EMR.
