We are seeking a dedicated Physical Therapist for a SNF in Virginia, Minnesota. This full-time role offers an hourly rate up to $50 and involves patient evaluations, treatment planning, and collaboration with a care team to restore mobility and manage pain.

Responsibilities include administering therapeutic exercises, educating families, monitoring progress, and maintaining documentation. Minnesota licensure and 1 year SNF experience are preferred.

Formación

• Licensed Physical Therapist in Minnesota is required.
• Minimum of 1 year experience, preferably in SNF.
• Strong communication and interpersonal abilities.
• Ability to work independently and with a team.

Responsabilidades

• Conduct initial evaluations and develop individualized treatment plans.
• Administer therapeutic exercises to improve strength and mobility.
• Use modalities to relieve pain and promote healing.
• Educate patients and families on home exercises for progress.
• Monitor progress and adjust treatment plans as needed.
• Collaborate with doctors and occupational therapists for comprehensive care.
• Maintain accurate documentation of patient progress and plans.
