Qualifications

• Bachelor's degree in Physical Therapy from an accredited college or university.
• Active, unencumbered license granted by the state of employment.
• Basic computer skills and ability to navigate service delivery software and electronic health records.

Responsibilities

• Return patient to optimal physical function within a SNF setting.
• Instruct patients in continuing exercises and activity modification post-discharge.
• Educate patients and families to support ongoing independence and goals.

Education

Bachelor's degree in Physical Therapy

Active license in state of employment

Basic computer skills and EHR familiarity
